If your passport is still valid and has been damaged, then a damaged passport application is the correct application for you. Your new passport will be valid for 10 years. Please note that you are responsible for paying the government fee of $244.53 for your damaged passport replacement.

Description / Damaged Passport

Damaged Passport Application. Damaged Passport Applications are for US citizens that are currently holding a still valid damaged US passport. Understand that once your passport became damaged it lost its power as a document, so you will need to also provide your certified birth certificate, naturalization certificate, or an old passport still in good condition.
