Proof of U.S. Citizenship
Original document required — include ONE of the following

Note: Texas and California "Abstract" birth certificates are not acceptable and will be rejected. - An expired U.S. Passport (must be in good condition)

Photocopy of Valid Photo ID — Front & Back
Color copy required — both sides

A color photocopy is strongly recommended for best results. A high-quality black & white copy may be accepted, but color is preferred and less likely to cause delays.
- One clear color copy — front and back — of your valid Driver's License, State ID, Military ID, or valid Passport
- If your name has changed from your citizenship document, include the original marriage certificate, divorce decree, or court-issued name change document

Two Passport Photos
Brand new, under 3 months old — Staples, CVS, Walgreens, etc.

Do not staple the photos to the application. Let the acceptance agent attach them. Stapling causes rejection.

- No hat or glasses in the photo — will be rejected

Letter of Authorization
Print, complete by hand in blue or black ink

- Downloaded and printed the Letter of Authorization
- Only the first 2 boxes at the top are checked — do not check the 3rd box
- Courier Company Name at the bottom is filled in as Urgent Passport
- Form is completed in blue or black ink (no pencil, no typed text)

DS-11 Form — Completed Online & Printed
Must be digitally typed and printed with barcode — handwritten NOT accepted

Handwritten DS-11 forms will not be accepted and cause a 4-day delay. You must complete the form online and print the barcoded PDF.
Follow these steps exactly on the State Department website. Watch the video guide if you're unsure about any step.

Proof of International Travel Departure
Include ONE of the following

You need documentation showing you have an upcoming international trip. Choose the option that applies to your travel:
- Flying internationally: One clear photocopy of your international plane ticket confirmation with your name listed as passenger
- Driving across border: A printed foreign hotel confirmation plus a typed letter explaining the ground travel
- Business travel: A typed company letter explaining the travel purpose

Government Passport Fee — $113.36
Personal check or money order payable to "U.S. Department of State" + $35 to acceptance agent

The State Department is very strict about payment. Illegible writing, abbreviations, scratch-outs, starter checks, or post-dated checks will cause a 4-day delay. Read the rules below carefully.
You need two separate payments: $113.36 by check or money order (goes inside the sealed envelope) and $35.00 cash paid directly to the acceptance agent at the time of your visit.
- Check or money order is payable to U.S. Department of State
- Applicant's date of birth is written in the memo line
- If using a money order: your name, address, and date of birth are all written on it
- If using a personal check: your name and address are printed on the check (not handwritten) — starter checks are rejected
- Made a photocopy of the completed check — this copy goes loose inside your FedEx/UPS shipping envelope
- Have $35.00 ready to pay the acceptance agent in person

Print Your Order Confirmation Email
Include loose in shipping envelope — do NOT let acceptance agent seal this
