Lost or Stolen Adult Passport Checklist
Complete every step below before visiting your acceptance agent. Check off each item as you go — your progress is saved in this browser session. When finished, call us to verify before shipping.

A certified official U.S. Birth Certificate
Texas and California "Abstract" birth certificates are not acceptable and will be rejected. - An expired U.S. Passport – must be the passport previous to the one you lost, and must be in good condition
- Original U.S. Naturalization Certificate, FS-240, DS-1350, or Certificate of Citizenship Abroad
- One clear color copy — front and back — of your valid Driver's License, State ID, Military ID, or valid Passport
- If your name has changed from your citizenship document, include the original marriage certificate, divorce decree, or court-issued name change document
- Two brand-new passport photos, taken within the last 3 months
- No hat or glasses in the photo — will be rejected
- Downloaded and printed the Letter of Authorization
- Only the first 2 boxes at the top are checked — do not check the 3rd box
- Courier Company Name at the bottom is filled in as Urgent Passport
- Form is completed in blue or black ink (no pencil, no typed text)
Follow these steps exactly on the State Department website. Both the DS-11 and DS-64 will print as a single PDF. Watch the video guide if you're unsure about any step.

If you do not know the lost passport book number, enter the digit "1" nine times to satisfy that field. -
On the "Products & Fees" page, selected:

Total should be $248.36 — verify this before clicking Next - On the "Electronic Signature" page, select "Print, Sign, and Mail"
- On the "Next Steps" page, scrolled to the bottom and clicked PRINT FORM
- Printed the PDF — it will contain both the DS-11 and DS-64 as a single document (do not handwrite anything on it)
You need documentation showing you have an upcoming international trip. Choose the option that applies to your travel:
- Flying internationally: One clear photocopy of your international plane ticket confirmation with your name listed as passenger
- Driving across border: A printed foreign hotel confirmation plus a typed letter explaining the ground travel
- Business travel: A typed company letter explaining the travel purpose
You need two separate payments: $213.36 by check or money order (goes inside the sealed envelope) and $35.00 cash paid directly to the acceptance agent at the time of your visit.
- Check or money order is payable to U.S. Department of State
- Applicant's date of birth is written in the memo line
- If using a money order: your name, address, and date of birth are all written on it
- If using a personal check: your name and address are printed on the check (not handwritten) — starter checks are rejected
- If also applying for a passport card, add $30.00 to the check (total $243.36)
- Made a photocopy of the completed check — this copy goes loose inside your FedEx/UPS shipping envelope
- Have $35.00 ready to pay the acceptance agent in person
- Found the order confirmation email from Urgent Passport Services
- Printed the PDF attached to that email (it contains your shipping addresses and order info)
- Keeping this printed page separate and loose — NOT inside the acceptance agent's sealed envelope
