If your name has changed since your passport was issued, AND your passport is LESS than a year old, you can apply for a re-application of your passport. This situation is fairly rare. If your passport was issued over a year ago and you need to change your name on your passport, you must apply for a fast passport renewal.

Passport Reapplication: Is this what you need? Ask yourself these questions:

• Was your passport issued within the last year?

If yes – you need a Re-Application Passport
If your passport was issued more than one year ago, and is still valid – you need a Renewal Application

Part 1 is the Urgent Passport Order Form and Letters of Authorization, Part 2 is the government passport application. Both parts are required and must be printed and completed in ink.
