Passport Renewals are for passport applicants that were issued an ADULT passport within the last 15 years. So if your passport was valid for 10 years, and it has expired within the last 5 years, a passport renewal is what you need. • Were you 16 or older when your most recent passport was issued to you?

• Enter all information requested by State Department (red asterisk fields are required)
• “Summary of Fees” page (only check $60 Expedite Fee & Passport Book Fee) & click SUBMIT
• Next Page – Scroll to bottom and click CREATE FORM
• Your completed Passport Application form will open as a PDF document, PRINT IT OUT & SIGN IT
