New Passports: Is this what you need? Ask yourself these questions:

Am I applying for a passport for the first time and I am 16 or older?
Was my prior passport issued at age 15 or under?
Was my prior passport issued more than 15 years ago?

If you answer yes to (Any) of these questions, then PRINT instructions 1 and 2 below.

Each part will open in a separate browser window. Part 1 is the Urgent Passport Order Form and Letter of Authorization, Part 2 is the government passport application. The U.S. State Department requires passport applicants to electronically complete and print Part 2. The completed form will print out with a barcode (Required by Government).
