Each part will open in a separate browser window. Part 1 is the Urgent Passport Order Form and Letter of Authorization, Part 2 is the government passport application. The U.S. State Department requires passport applicants to electronically complete and print Part 2. The completed form will print out with a barcode (Required by Government). Part 3 is the statement explaining the damage to your passport. All three parts are required.
