Child Passport: Is this what you need? Ask yourself these questions:

Are you age 15 or younger?

If you are age 15 and Under, you need to follow these instructions. PRINT instructions 1, 2, and 3 below. All three parts are required. If the child is 15 or younger, they should file for this type of application. There is no such thing as a renewal for a child.

Each part will open in a separate browser window. Part 1 is the Urgent Passport Order Form and Letter of Authorization, Part 2 is the government passport application. The U.S. State Department requires passport applicants to electronically complete and print Part 2. The completed form will print out with a barcode. Part 3 is the non-present parental consent.
